Must be registered as a patient to receive services. There may be a cost for new patient registration. Comprehensive primary care produces better health outcomes among patients and lowers health care costs. PCC offers a wide-range of primary care services to patients of all ages, and no one is denied services because of inability to pay. PCC accepts many major health plans, Mediciad, Medicare, as well as offers a sliding fee discount program for patients who do not have insurance. Services provided include: Pediatric and Adolescent health care that includes well-child exams, immunizations, sick infant/child exams, school and sports physicals, and lead screenings; Womens Health Care (ob/gyn) that includes maternity care and delivery, Pap smears, and breast cancer screenings; Adult Health Care; Senior Health Care; Diagnosis and treatment of illness; disease screening and prevention; family planning (birth control); management of chronic illnesses such as asthma, diabetes, and high blood pressure; and HIV testing and counseling. You do not need to be registered as a patient in order to set up an appointment for a free rapid HIV test; however only the PCCs Lake Street location has routine HIV testing (14 Lake St., in Oak Park, IL 60302- & the phone number for the Oak Park location is: (708) 383-0113). |
