Services are for adults. Is an HIV clinic of the Cook County Health and Hospital System. Comprehensive Medical Case Management services are offered to clients who are eligible for services. Case Management services are designed to foster a client's sense of price, independence, and self-empowerment. This relationship holds the client/case manager responsible for the service plan. The goal of case management is to coordinate care requested by the client and to keep clients in medical care and taking their medications.
